Photo taken by Hugh Halliday in 2009 in multi-faith military chapel at Uplands airfield. A Polish-Canadian artist, Mr. A. Birkenmeyer, had been commissioned to produce two plaques: one in Ottawa and the other for Warsaw.

Photo (provided by Hugh Halliday) was taken at unveiling of plaque 25 Novermber 1946, at the Military Hospital, 27 Nowowiejska Street, Warsaw.  A Polish-Canadian artist, Mr. A. Birkenmeyer, had been commissioned by the United Polish Relief Fund of Canada to produce two plaques, one in Polish.
